چکیده

High tibial osteotomy can provide 10 to 15 years of consistent pain relief and improved function for most patients with unicompartmental knee arthritis. However, some studies have reported less predictable results, with deteriorating survivorship after 10 years of follow-up. Therefore, it is paramount to carefully select patients and to understand that technical errors during surgery often lead to poor outcomes. It is well established that obese patients, as well as undercorrected osteotomies, often lead to early failure, with rapid degeneration of the arthritic compartment. Although many surgical techniques describing high tibial osteotomies have been previously published, most techniques share similar surgical principles. What differentiates our technique from others is the mode of harvesting local autograft bone to fill our osteotomy site. For several years, a specialized harvester has been used to obtain distal femoral cancellous bone. This technique is simple, efficient, reproducible, and safe. This article reviews the novel technique for opening wedge high tibial osteotomies.

The clinical Outcome of One-stage High Tibial Osteotomy and Anterior Cruciate Ligament Reconstruction. A Current Concept Systematic and Comprehensive Review

Background: Patients with an anterior cruciate ligament-deficient varus-angulated knee may need not only an isolated high tibial osteotomy (HTO), but also an additional anterior cruciate ligament reconstruction (ACLR). A number of prospective clinical trials have been published considering the combination of HTO and ACL reconstruction. Our aim was to investigate whether one-stage combined HTO...

The Outcome of Bone Substitute Wedges in Medial Opening High Tibial Osteotomy

BACKGROUND Opening wedge high tibial osteotomy often requires bone grafting to improve the union rate and avoid instability at the osteotomy site. Autograft and allograft have both been associated with complications and the use of bone substitute wedges has been advocated to improve the outcome. This study investigated the clinical, radiological and histological outcomes of using biphasic calci...

High tibial osteotomy: closed wedge versus combined wedge osteotomy

BACKGROUND High tibial osteotomy is a common procedure to treat symptomatic osteoarthritis of the medial compartment of the knee with varus alignment. This is achieved by overcorrecting the varus alignment to 2-6° of valgus. Various high tibial osteotomy techniques are currently used to this end. Common procedures are medial opening wedge and lateral closing wedge tibial osteotomies. The latera...

Fixation stability of opening- versus closing-wedge high tibial osteotomy: a randomised clinical trial using radiostereometry.

Valgus high tibial osteotomy for osteoarthritis of the medial compartment of the knee can be performed using medial opening- and lateral closing-wedge techniques. The latter have been thought to offer greater initial stability. We measured and compared the stability of opening- and closing-wedge osteotomies fixed by TomoFix plates using radiostereometry in a series of 42 patients in a prospecti...

Combination of bone marrow derived cells transplantation and high tibial osteotomy in early osteoarthritis of knee: A preliminary study

Purpose: high tibial osteotomy (HTO) is a recommended treatment for medial compartment knee osteoarthritis. Newer cartilage regenerative procedures may add benefits to the result of HTO. In this prospective study we investigate safety and also results of HTO associated with bone marrow derived cells (BMDC) transplantation in relatively young and middle aged active individuals with early osteoar...
